Trust led asssessment services at Christchurch Day Hospital

At Christchurch Hospital an all age rehabilitation service has been developed, particularly in the award-winning Day Hospital. Most patients are however elderly, reflecting the local population. There is an excellent infrastructure to support rehabilitation with superb physiotherapy and occupational therapy facilities.

The purpose of the service is to:

- Provide specialist assessment and rehabilitation for older people presenting with acute and long term conditions.

- Admission avoidance.

- Facilitate early supported discharge.

- Support and assessment of patients to enable them to remain in their own homes.

- Signposting to appropriate intermediate care services for further support.

The services are managed by the RBCH NHS Foundation Trust and form part of the wider intermediate care structure covering patients living in the catchment areas of NHS Bournemouth & Poole & NHS Dorset.

The Day Hospital service offers:

- Community Falls team reviews and treatment planning

- Memory Clinics

- MDT assessments and rehabilitation programmes

- Balance groups

- Cardio Vascular service for Stroke patients

- Parkinson’s groups

- FES Clinic – Functional Electrical Stimulation

- Vestibular Interventions Clinic

- Ear Irrigation

In addition to experienced occupational therapists and physiotherapists, there is also a nursing team who specialise in the older person and multipathological conditions which patients present with. Each patient attending the Day Hospital is also allocated a consultant.

We want to ensure that we continue to provide NHS service from Christchurch Hospital. Over the last two years, through public consultation, we have been developing plans to ensure the hospital remains a focal point for the local community.

We want your hospital stay to be the best that it can be. Ensuring that you have the best care and treatment while you are on our ward is really important to us.

If you are not happy with the care that we are providing please ask for the nurse in charge. We want to be able to answer and address your questions and concerns as soon as possible and improve your experience, or that of your relative, while you are here.
